Responsibilities
- Act as the initial contact for incoming referrals related to IDD and housing support services
- Accurately and promptly process intake forms and supporting documentation
- Perform intake assessments, confirm eligibility criteria, and ensure file completeness
- Collaborate with internal staff, case management personnel, and outside agencies
- Adhere to regulatory requirements and standards specific to IDD and housing programs
- Interact with families, support planners, and external parties using professional communication
- Balance multiple tasks while demonstrating precision, care, and organizational skills
