Grace Community Care and Homes Inc. is hiring an Intake Coordinator to join our team. In this role, you will serve as the first point of contact for new IDD and housing support referrals. We need a high-integrity individual who thrives in a fast-paced environment and values precision, professionalism, and compassion in their work.
What You'll Do
- Serve as the first point of contact for new IDD and housing support referrals.
- Process and manage intake documentation accurately and efficiently.
- Conduct intake screenings, verify eligibility, and ensure completeness of records.
- Coordinate with internal teams, case managers, and external partners.
- Maintain compliance with IDD and housing support regulations and standards.
- Communicate professionally with families, support coordinators, and stakeholders.
- Manage multiple priorities while maintaining attention to detail and empathy.
What We're Looking For
- Minimum of 3 years experience in medical intake, healthcare administration, IDD services, or customer service in a regulated or professional environment.
- Strong understanding of confidentiality, ethics, and integrity.
- Exceptional communication and organizational skills.
- Ability to work efficiently in a fast-paced, high-volume environment.
- Comfortable with technology and learning new systems quickly.
- Professional demeanor and judgment at all times.
- Must have high-speed, reliable internet.
- Ability to use their own device when working remotely.
- Must be dependable, punctual, and able to manage time independently.
