Grace Community Care and Homes Inc. is hiring an Intake Coordinator

Responsibilities

  • Act as the initial contact for incoming referrals related to IDD and housing support services
  • Accurately and promptly process intake forms and supporting documentation
  • Perform intake assessments, confirm eligibility criteria, and ensure file completeness
  • Collaborate with internal staff, case management personnel, and outside agencies
  • Adhere to regulatory requirements and standards specific to IDD and housing programs
  • Interact with families, support planners, and external parties using professional communication
  • Balance multiple tasks while demonstrating precision, care, and organizational skills

Monday–Friday, 9:00 AM – 5:00 PM
