Northwestern Memorial Healthcare is hiring a Casual ICC Patient Flow Coordinator for its Immediate Care System. In this role, you will virtually triage the waiting rooms of high-volume Immediate Care Centers, initiate clinical protocols, and improve patient throughput. You'll work 12-hour shifts every two weeks (7:15am-7:45pm), including weekends and holidays.
What You'll Do
- Determine and communicate each assigned ICC location's daily operational status and update the Epic message log.
- Establish a positive first impression with patients via phone, maintaining privacy and confidentiality according to NM standards.
- Monitor assigned regional ICC waiting rooms through Epic ED Manager based on patient complaint and wait time.
- Demonstrate proficiency in timely and accurate documentation within the patient's electronic medical record (EMR).
- Determine patient eligibility for specific ICC care protocols based on arrival chief complaint and telephone evaluation.
- Direct appropriate resources quickly based on symptom severity and recognize emergent patients, alerting the in-clinic team.
- Document conversations, analyze patient health concerns, and follow a standard process of care to assign diagnostics.
- Communicate the plan of care and diagnostic testing to patients per protocol and place orders in Epic for lab, x-ray, and cardiology.
- Know each regional ICC department's locations, patient flow processes, and radiology services.
- Communicate efficiently with ICC staff via track board and verbally to activate orders and coordinate specimen collection, EKGs, and x-rays.
- Assist in coordinating Telemedicine Conversion patients, assigning them to a provider and adding diagnosis codes.
- Place orders based on symptoms per ICC protocol when a patient consents to a Telemedicine Conversion during triage.
- Update telehealth providers via the Epic track board to facilitate timely visits and communicate with staff using Microsoft Teams.
- Travel to the closest Northwestern facility if home equipment is nonfunctional.
- Participate in new employee orientation, provide feedback, and attend staff meetings and huddles.
What We're Looking For
- Current Registered Nurse License issued by the State of Illinois.
- American Heart Association Basic Life Support CPR certification.
- BSN or higher degree.
- Previous Emergency Department or Urgent Care triage experience.
- Required Emergency Department or Urgent Care Nurse experience.
Nice to Have
- At least 3 years’ experience in an emergency department setting.
- TNCC or TNS Certification.
- Proficiency with the Epic electronic charting system.
- Immediate care background.

Work Mode
This is a hybrid position. You may work remotely from home or be based at our Glenview, IL location or one of our 25 ICC locations.
