Vytalize Health is hiring a Health Coach to serve as a dedicated patient support person within an interdisciplinary clinical team. You will focus on empowering patients with complex medical needs to actively manage their health, helping them gain the knowledge, skills, tools, and confidence to become active participants in their care and reach their self-identified goals.

What You'll Do

Serve on an interdisciplinary care team as patient health coach, in partnership with a clinical RN and Pharmacist.
Actively engage in patient rounds and act as a facilitator of change for assigned patients.
Empower patients to set their own health and wellness goals and offer support as they make progress.
Support patients as they learn to recognize their own values, internal strengths, and insights, and mobilize those assets.
Offer support to develop self-management strategies to help patients make sustainable changes.
Help patients understand their motivation for change and empower them to see themselves as experts on their own health.
Provide support and accountability for quantifiable goal setting.
Use broad knowledge of health and wellness to help clients navigate through their health concerns.
Document services provided in accordance with program and compliance requirements.
Serve as a liaison and partner to patient caregivers and collaborate with healthcare providers as needed.
Provide remote telephonic coaching services at a frequency based on risk, need, and program specific workflows.

What We're Looking For

Minimum of 3 years’ experience in chronic condition clinical coaching (Frailty, Diabetes, CHF, COPD, etc.) or other relevant healthcare coaching experience.
National Board for Health and Wellness Coaching (NBHWC) Certified or willing to be certified within 3 months of employment.
Excellent listening, written, and verbal communication skills.
Empathy, emotional intelligence, and the ability to build strong, trusting relationships with patients.
Deep understanding of positive psychology, behavior change, and motivation.
Mindfulness skills, coaching presence, and problem-solving skills.
Demonstrate a positive attitude and respectful, professional customer service.
Acknowledge patient’s rights on confidentiality issues and follow HIPAA guidelines and regulations.
Comfort in interacting as part of an interdisciplinary team, with confidence in speaking with physicians and medical staff.
Comfortable with digital technology and able to troubleshoot technology issues.
Ability to critically think, solve problems, and bring professionalism to all situations.
Able to contribute to quality improvement and process improvement initiatives.
Ability to be flexible and embrace change while remaining highly organized, and detail oriented.

Nice to Have

Bachelor’s degree in healthcare or a related field.
