CL Healthcare Inc. is looking for a Community Health Nurse to provide skilled nursing care and assessments to PACE members under the direction of a Nurse Manager. In this role, you will conduct visits primarily in members' homes, while also coordinating care in centers, hospitals, and other settings as part of the core interdisciplinary team.
What You'll Do
- Carry out the agency’s mission, philosophy, goals, and objectives.
- Assess and evaluate the health care needs of patients and families considering physiological, psychological, social, and environmental factors.
- Identify complete, accurate, and logical Uniform Assessment System (UAS) for NY assessments in collaboration with the primary care physician.
- Participate in mandatory PACE comprehensive care planning meetings as required by regulations.
- Collaborate with all members of the interdisciplinary team to develop a comprehensive care plan based on member-specific needs, physician orders, and identified goals.
- Implement and revise nursing care plans by regularly assessing, observing, and evaluating the patient’s condition, needs, and response to care.
- Initiate and apply appropriate preventative, therapeutic, and rehabilitative nursing procedures and techniques.
- Administer medications and treatments as prescribed and independently perform skilled nursing procedures.
- Teach patients and family/caregivers self-techniques and provide instruction regarding medication, diet, safety, and treatment modalities.
- Provide health counseling and education to patients, families, and other caregivers.
- Delegate responsibility appropriately and supervise ancillary personnel.
- Evaluate and promote a safe environment for the patient.
- Complete, maintain, and submit all required documentation that is timely, accurate, and relevant.
- Perform UAS-NY assessor functions including timely completion, follow-up assessments, and significant change assessments.
- Review and confirm member eligibility for the PACE program based on the Nursing Facility Level of Care score.
- Perform care coordination as part of nursing visits across all settings of care, collaborating with internal and external health care professionals.
- Act as a member of the interdisciplinary team.
What We're Looking For
- Degree from an NLN-accredited school of nursing.
- Minimum of one (1) year general clinical nursing experience in a certified home health agency, acute care, medical surgical, and/or critical care.
- Clinical nursing experience must be within one (1) year from date of hire or transfer.
- Minimum of one (1) year of experience working with a frail or elderly population.
- Possess and maintain good physical stamina and mental health, including satisfactory health clearance.
- Current Driver’s License, required insurance, and car available for work.
- Current New York State license to practice as a Registered Nurse.
- Legal authorization to practice in New York State.
- Medical clearance for communicable diseases and all immunizations up-to-date.
- Computer and laptop proficiency.
- Good organizational, observation, communication, and judgment skills.
- Self-directed with ability to work with supervision.
- Flexible and cooperative in fulfilling role obligations.
- Ability to meet a standardized set of competencies for the specific position established by the PACE organization before working independently.
Nice to Have
- BSN preferred.
Team & Environment
You will be a core member of the interdisciplinary team, reporting directly to a Nurse Manager.
Benefits & Compensation
- Salary: $105,861.41 - $118,061.41
