About the Role

MDH Network seeks an experienced Community Activities Specialist to lead person-centered assessments and strengthen staff capability across our Coordinated Family Supports (CFS) and Coordinated Life Services (CLS) programs. This central role ensures Individuals receive high-quality, individualized services while equipping staff with the tools and skills needed for success. The position combines hands-on assessment work with structured training, coaching, and field support.

What You'll Do

  • Conduct person-centered assessments and develop individualized service recommendations aligned with each Individual's goals and needs.
  • Collaborate with Individuals, families, and team members during intake and service planning.
  • Monitor service delivery to ensure alignment with planned supports and maintain ongoing team communication.
  • Deliver structured training and on-the-job coaching for new and existing staff.
  • Coach staff in person-centered practices, documentation standards, communication, health and safety, and behavioral supports.
  • Conduct 30- and 60-day field evaluations to assess competency and address performance gaps.
  • Interview Individuals to assess service satisfaction and effectiveness.
  • Review documentation for accuracy and compliance, identifying trends and recommending improvements.

What We're Looking For

  • Minimum two (2) years of experience working with Individuals with intellectual or developmental disabilities, including behavioral challenges or dual diagnoses.
  • Strong understanding of person-centered practices.
  • Ability to conduct structured assessments and translate findings into practical service recommendations.
  • Strong written and verbal communication skills.
  • Effective time management and ability to balance field and office responsibilities.
  • Comfortable working independently across multiple locations.
  • High School Diploma plus an associate degree in psychology, behavioral science, or a related field.
  • Non-Violent Crisis Prevention certification (or ability to complete training).
  • Current CPR and First Aid certification (or ability to complete training).
  • Direct Support Professional I & II Certification (or ability to obtain within six months of hire).

Nice to Have

  • Experience training, mentoring, or coaching staff.
  • Bachelor's degree in psychology, special education, or a related field.

Work Mode

This is a hybrid role based in Southern California, requiring a mix of field work, office time, and remote coordination.
