Responsibilities
- Ensuring that patient care is coordinated and managed appropriately.
- Serving as the first point of contact for field clinicians to ensure support is easily accessible in a timely fashion.
- Ensuring that care and services are delivered appropriately as well as the supervision of clinical personnel.
- Oversight of clinicians providing patient care to patients in their place of residence.
- Handling incoming calls timely, and follows up as necessary to ensure appropriate actions are taken to meet the internal and external customer needs.
- Coordinating training and educational programs for nurses within the field to improve clinical outcomes and competencies.
- Assessing staff learning needs and formulates specific plans and goals for next evaluation period with employees, performs administrative call duties, and serves as a role model and resource for staff.
- Assisting in patient care escalations etc.
- Coordinating at least bi-weekly Case Conferences with clinical team to discuss patients' progress and for revision of the care plan as needed to meet planned outcomes.
- Reviewing and evaluating each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
- Reviewing patient’s clinical diagnosis, medications, procedures and clinical course.
- Assisting clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.
Requirements
- Two years management or supervisory experience in a rehabilitation therapy department or program, preferably home care.
- Degree from a baccalaureate or masters program in applicable discipline approved by an accredited organization.
- Current CA Physical Therapist license
- Current Basic Life Support (BLS)
- Valid CA Drivers License with acceptable driving record
- All license and certifications must be current at time of hire and sustained throughout employment

- All opportunities at 21HHS require being in the field visiting patients in their homes. Remote/work-from-home positions are not available unless specifically noted.
- Coverage area: Santa Rosa (Sonoma County)

- You will be assigned a primary location; however, you may occasionally be asked to provide coverage outside of that area. This may include supporting vacancies, leaves of absence, higher patient census, or other client care needs. While 21st Century Home Health makes every effort to maintain your primary territory, temporary reassignments may be necessary to ensure safe, high-quality patient care and to meet the needs of our growing organization.
