Shield AI is looking for a Clinical Social Worker (MSW or LCSW) to provide essential medical social work services in a home health environment. This is a per diem position where you will support patients directly in their homes.
What You'll Do
- Provide professional medical social work services to patients in a home health setting.
- Conduct assessments and develop care plans to address patient and family psychosocial needs.
- Collaborate with interdisciplinary healthcare teams to ensure integrated patient care.
- Offer counseling and crisis intervention services as required.
What We're Looking For
- Valid Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW) credential.
Work Mode
This is a hybrid position based in Santa Rosa, California. The role combines in-person home health visits with remote administrative duties.
