Title: Clinical Call Center Triage Nurse Company: Oak Street Health Location: Remote Role: The Clinical Call Center Triage Nurse extends patient care beyond regular clinic hours, addressing medical, social, and psychological needs via telephone. Core Responsibilities: • Provide confident and professional service to OSH patients • Follow current Prescription Refill policy • Remain fully engaged with patients, minimizing background distractions • Maintain clear and precise documentation during calls • Complete all patient request documentation before ending each call • Monitor and complete CareReq queue • Handle all tasks within the role's scope and ability • Schedule appointments for patients needing off-cadence or additional visits • Arrange patient transportation for clinic visits • Stay aligned with organizational goals and departmental metrics • Perform other duties as assigned Career Development Opportunities: The progression from Clinical Call Center Triage Nurse I to II includes: • Minimum 6-month tenure in Triage RN I role • Consistently demonstrate: - Strong problem-solving skills - Effective communication - Thorough understanding of patient needs • Show genuine desire to learn and grow • Introductory learning of key performance metrics: - Average Unavailable Time - Average Handle Time - Average Calls per Hour - Quality Metrics - Care Req Reduction - CET Management • Proven reliability and satisfactory attendance This structured path encourages nurses to expand their knowledge, take on more responsibility, and showcase expertise – ultimately contributing to superior patient support.
