Hazel Health is hiring a Bilingual Physical Health Support Specialist

About the Role

The specialist supports physical health initiatives by delivering care coordination, health education, and multilingual support to students and their families through virtual platforms.

Responsibilities

  • Deliver health guidance to students and caregivers in both English and Spanish
  • Conduct remote check-ins to monitor student recovery and symptom progression
  • Collaborate with school staff to facilitate student health accommodations
  • Document patient interactions accurately in the electronic health record system
  • Support implementation of care plans developed by medical providers
  • Respond to family inquiries about physical health concerns during school hours
  • Assist in triaging student-reported symptoms for appropriate follow-up
  • Educate families on managing common illnesses and injuries at home
  • Coordinate with nurses and clinicians to ensure continuity of care
  • Maintain confidentiality and compliance with privacy regulations
  • Follow up on lab results and treatment outcomes as directed
  • Help students understand medication instructions and treatment steps
  • Identify social determinants affecting student health and escalate appropriately
  • Participate in team meetings to review case updates and protocols
  • Support outreach efforts to improve student engagement in care programs
  • Use telehealth tools to communicate with patients and guardians
  • Track patient progress using digital health management systems
  • Report urgent health issues to supervising clinicians immediately
  • Assist in translating medical instructions when needed
  • Ensure timely communication between school, family, and clinical team
  • Follow standardized protocols for common pediatric health conditions
  • Maintain professional boundaries in all patient interactions
  • Adapt communication style for developmental appropriateness
  • Contribute to quality improvement initiatives in student health services
  • Stay current with clinical guidelines and organizational procedures
