About the Role
The Clinical Nurse Navigator provides personalized care coordination for patients, leveraging bilingual abilities to improve access, education, and health outcomes across diverse populations.
Responsibilities
- Guide patients through healthcare systems from diagnosis to treatment
- Conduct assessments to identify patient needs and barriers to care
- Deliver culturally appropriate health education in both English and Spanish
- Coordinate referrals to specialists and support services
- Monitor patient progress and adjust care plans as needed
- Serve as a primary point of contact for patients and families
- Document patient interactions and care plans in electronic records
- Advocate for patient needs within the healthcare team
- Facilitate communication between providers and non-English-speaking patients
- Support medication adherence and treatment understanding
- Assist with scheduling diagnostic tests and follow-up visits
- Identify social determinants of health impacting care access
- Connect patients with community resources and financial assistance
- Promote preventive care and early intervention strategies
- Collaborate with interdisciplinary teams to ensure continuity of care
- Participate in quality improvement initiatives
- Respond to patient inquiries via phone, email, or in person
- Ensure compliance with privacy and healthcare regulations
- Track patient outcomes and report data for performance metrics
- Maintain current knowledge of clinical guidelines and resources
- Support patient transitions between care settings
- Educate patients on managing chronic conditions
- Assist with pre-authorization and insurance navigation
- Contribute to patient satisfaction and retention efforts
- Uphold standards of patient dignity and informed consent
