About the Role
This role involves delivering comprehensive case management services to individuals with mental health and substance use challenges, ensuring access to care and promoting recovery through regular assessments, care planning, and multidisciplinary collaboration.
Responsibilities
- Conduct initial and ongoing assessments of client needs
- Develop individualized care plans in collaboration with clients
- Coordinate access to mental health and social services
- Monitor client progress and adjust care strategies
- Engage clients through outreach and motivational techniques
- Maintain accurate and timely case documentation
- Collaborate with clinical and community partners
- Facilitate transitions between care settings
- Support clients in achieving recovery goals
- Conduct home and community-based visits as needed
- Respond to client crises with appropriate interventions
- Ensure compliance with program standards and regulations
- Participate in team meetings and case reviews
- Provide education on mental health and wellness
- Assist clients with housing and employment resources
- Advocate for client needs within healthcare systems
- Use technology for scheduling and record keeping
- Follow up on missed appointments and engagement barriers
- Promote client independence and self-advocacy
- Maintain professional boundaries and confidentiality
Nice to Have
- Master’s degree in social work or counseling
- Licensed or license-eligible in relevant field

- Bilingual in Spanish and English
