CareHarmony is seeking a Licensed Practical Nurse (LPN) to join our team as a remote Intake Coordinator. You will work comprehensively with healthcare providers to deliver value-based care management, focusing on closing gaps in patient care and coordinating essential services.
What You'll Do
- Manage a patient census with a resolution-driven approach to close gaps in clinical and non-clinical care.
- Identify and coordinate community resources to benefit patient care plans.
- Provide patient education and improve health literacy on the management of chronic conditions.
- Perform medication management, including reconciliation, adherence tracking, and coordinating refills.
- Assist in ensuring timely delivery of critical services like Home Health, DME, and Home Infusion.
- Resolve patient questions and create an open dialogue to understand their needs.
- Assist in managing referrals and scheduling appointments.
What We're Looking For
- Active Washington D.C. License & active Compact/Multistate License (LPN).
- At least 3 years of direct patient-facing work experience.
- Technical aptitude with the Microsoft Office Suite.
- Excellent written and verbal communication skills.
- Must have active high-speed Wi-Fi.
- Must have a home office or HIPAA-compliant workspace.
Nice to Have
- Experience with Epic EHR software.
- Bilingual communication skills.
- Additional state LPN licensures.
