About the Role
The Registered Dietitian will provide medical nutrition therapy to patients using a digital health platform, focusing on chronic disease management and personalized care planning.
Responsibilities
- Deliver individualized nutrition assessments and counseling
- Develop and adjust nutrition care plans based on patient needs
- Collaborate with physicians and care coordinators
- Document patient interactions in the electronic health record
- Support patients with conditions such as diabetes, hypertension, and obesity
- Educate patients on dietary modifications and lifestyle changes
- Monitor patient progress and adjust interventions as needed
- Participate in care team meetings and case reviews
- Respond to patient messages within established timeframes
- Stay current with clinical guidelines and nutrition research
- Ensure compliance with privacy and regulatory standards
- Contribute to quality improvement initiatives
- Assist in developing patient education materials
- Engage in ongoing training and professional development
- Promote patient engagement and adherence to care plans
Nice to Have
- Master's degree in Nutrition or related field
- Certification in diabetes care or obesity management
- Experience with population health programs
- Background in behavioral health integration
- Familiarity with value-based care models
