Advanced Health Care of Albuquerque is hiring a Physical Therapist to play a crucial role in providing high-quality rehabilitation services to our residents. You will focus on restoring mobility, improving strength and function, and enhancing overall quality of life.
What You'll Do
- Evaluate residents' physical abilities, functional status, and rehabilitation needs through comprehensive assessments.
- Develop personalized treatment plans based on assessment findings, resident goals, and evidence-based practice guidelines.
- Implement therapeutic interventions to address impairments, maximize functional independence, and promote safe mobility.
- Provide one-on-one physical therapy sessions to residents, utilizing a variety of therapeutic techniques and modalities.
- Monitor resident progress and adjust treatment plans as needed to optimize outcomes and achieve rehabilitation goals.
- Collaborate with team members, including physicians, nurses, occupational therapists, and speech-language pathologists, to coordinate care and ensure continuity of services.
- Educate residents and their families on exercise programs, safety techniques, and home exercise regimens to support ongoing rehabilitation and prevent re-injury.
- Document all physical therapy services provided, including initial evaluations, progress notes, and discharge summaries, in accordance with facility policies and regulatory requirements.
- Participate in resident care conferences, team meetings, and in-service training sessions to share expertise, contribute to care planning, and stay informed about best practices.
- Contribute to a positive and supportive work environment by demonstrating professionalism, teamwork, and effective communication skills.
What We're Looking For
- Current state licensure or eligibility for licensure as a Physical Therapist in the state of New Mexico.
- Strong clinical reasoning and decision-making skills, with proficiency in evaluating and treating a diverse range of conditions and diagnoses.
- Ability to build rapport with residents through a compassionate and caring demeanor.
- Excellent interpersonal and communication skills, with the ability to collaborate effectively with residents, families, and team members.
- Commitment to providing high-quality, resident-centered care with empathy and compassion.
Nice to Have
- 0-3 years of clinical experience in a long term care, skilled nursing facility, or similar healthcare setting is preferred.
