About the Role
The physical therapist will evaluate patient conditions, develop individualized treatment plans, and implement therapeutic interventions during home visits to promote functional improvement and independence.
Responsibilities
- Conduct initial patient assessments in home settings
- Design customized treatment programs based on clinical evaluations
- Perform therapeutic exercises and manual techniques to improve mobility
- Monitor patient progress and adjust care plans as needed
- Document treatment outcomes and maintain accurate medical records
- Educate patients and caregivers on therapy techniques and safety
- Collaborate with physicians and other healthcare providers
- Ensure compliance with state and federal healthcare regulations
- Use clinical judgment to modify interventions based on patient response
- Provide training in the use of assistive devices
- Assess home environments for safety and accessibility
- Report changes in patient condition promptly
- Maintain professional licensure and certifications
- Adhere to infection control protocols during visits
- Participate in ongoing patient re-evaluations
Nice to Have
- Two or more years of physical therapy experience
- Experience in geriatric rehabilitation
- Certification in a specialty area of physical therapy
- Training in manual therapy techniques
- Experience using outcome measurement tools
