Sprinter Health is hiring a Patient Engagement Specialist I - BILINGUAL (Spanish)

Responsibilities

  • Assist patients in navigating healthcare services and resources.
  • Coordinate care plans and services with healthcare providers.
  • Educate patients on health management and wellness.
  • Facilitate communication between patients and healthcare teams.
  • Document patient interactions and updates in electronic health records.
  • Provide language support to Spanish-speaking patients.
  • Ensure patient satisfaction and address any concerns or issues.
  • Collaborate with multidisciplinary teams to improve patient outcomes.
  • Conduct outreach to engage patients in their care.
  • Monitor patient progress and provide necessary support.
  • Assist in scheduling appointments and follow-ups.
  • Promote health literacy and self-management skills.
  • Maintain confidentiality and adhere to HIPAA guidelines.
  • Participate in training and development opportunities.
  • Contribute to the development of patient engagement programs.
  • Support the implementation of care coordination strategies.
  • Assist in data collection and reporting for quality improvement.
  • Provide feedback to enhance patient engagement initiatives.
  • Engage with community resources to support patient needs.
  • Facilitate patient access to social services and support groups.
  • Participate in team meetings and case discussions.
  • Support the development of patient education materials.
