Responsibilities
- Place and receive a high volume of calls with patients, healthcare providers, and pharmacies using standardized guidelines, consistently meeting performance benchmarks such as call volume and quality metrics.
- Partner with patients to address challenges affecting medication adherence, including financial concerns, forgetfulness, or misunderstanding of treatment importance, and refer clinical issues to pharmacists when necessary.
- Use persuasive communication, empathy, and education to address patient hesitations and gain commitment to prescribed medication plans.
- Coordinate directly with healthcare providers or pharmacies to facilitate timely prescription renewals.
- Record all patient interactions, outcomes, and encountered obstacles accurately in designated tracking systems.
- Identify potential improvements in pharmacy-related workflows and operational processes.
- Engage in training programs and peer learning to deepen knowledge of pharmacy quality performance indicators such as STAR scores.
- Share insights and effective strategies, provide feedback, and support team members to help meet organizational objectives.
- Perform additional tasks as required by leadership.
