Responsibilities
- Work directly in clinics to assist with Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs.
- Engage with patients in person to review their care plans, answer questions, and ensure they are on track with their health goals.
- Serve as a liaison between patients, clinical staff, and Medsien’s care management team to ensure effective care coordination.
- Assist clinic staff with program enrollments, patient follow-ups, and ongoing care monitoring.
- Support physicians and clinical staff in providing seamless patient care by preparing relevant health data for patient visits.
- Work closely with Medsien's care management team to streamline care delivery and identify potential areas for process improvement.
Requirements
- Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification
- 1+ years of experience working in a clinical setting, preferably in a care management or chronic disease management role
- Strong communication skills and the ability to work collaboratively with clinical staff and patients
- Proficiency in medical terminology and clinical processes
- Must be highly organized, detail-oriented, and capable of handling multiple tasks in a fast-paced environment
- Willingness to travel to different clinic locations in the Greater Alexandria, Virginia area as needed
