About the Role
The Occupational Therapist will evaluate and treat patients in home settings, focusing on enhancing their ability to perform everyday tasks. This role emphasizes individualized care plans, patient progress tracking, and coordination with other health providers.
Responsibilities
- Conduct initial and ongoing assessments of patients' functional abilities
- Develop customized treatment plans to improve independence in daily activities
- Provide therapy services in patients' homes
- Monitor and document patient progress over time
- Adjust interventions based on patient response and goals
- Collaborate with physicians and caregivers to coordinate care
- Train patients and families in adaptive techniques and equipment use
- Ensure compliance with clinical and regulatory standards
- Maintain accurate and timely patient records
- Communicate changes in patient condition to supervising clinicians
- Promote safety and accessibility within home environments
- Assist with mobility, self-care, and cognitive skill development
- Implement strategies to prevent injury and support recovery
- Participate in team meetings and case reviews
- Respond to patient needs with professional judgment and empathy
- Adhere to infection control and safety protocols
- Manage schedule for multiple home visits daily
- Transport therapy supplies and equipment to appointments
- Support patient transitions between care levels
- Uphold patient confidentiality and ethical standards
Nice to Have
- Master's degree in Occupational Therapy
- One year or more of home health experience
- Experience with geriatric populations
- Training in neurological rehabilitation
- Certification in hand therapy
- Experience using OASIS assessments
- Knowledge of telehealth platforms
- Bilingual communication skills
