DaVita Integrated Kidney Care (IKC) seeks a Nurse Practitioner (NP) to join our team serving the Osceola County, FL region. In this full-time role, you will provide integrated, holistic care to some of our most complex kidney patients, helping them navigate the healthcare system and improve their quality of life.

What You'll Do

Work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care.
Complete Comprehensive Health Assessments.
Manage complex patients, focused on reducing hospitalizations and improving health outcomes.
Deliver holistic, integrated care to improve the quality of life for kidney patients.

What We're Looking For

Passion for delivering care to patients with complex, chronic illnesses.
A minimum of two (2) years' experience as a Nurse Practitioner (NP).
Current unrestricted NP license to practice in the state.
Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
Intermediate computer skills and proficiency in MS Word, Excel & Outlook.
Home Office, High Speed Internet and Reliable Transportation Required.

Nice to Have

Experience working in Value Based Care or Population Health, including documenting and coding from a Medicare Advantage risk perspective.
5+ years experience as a Family Nurse Practitioner, Primary Care NP, or experience in Internal Medicine, Cardiology, Nephrology, or Case Management.
Spanish-speaking proficiency.
Experience in managing chronic diseases such as CHF, diabetes, and hypertension.
Experience managing population health with a focus on hospitalization.
