About the Role
The selected candidate will be responsible for conducting patient evaluations, developing individualized care plans, and coordinating treatment with other healthcare professionals to ensure optimal health outcomes.
Responsibilities
- Perform comprehensive health assessments for new and existing patients
- Diagnose and treat acute and chronic medical conditions
- Order and interpret diagnostic tests including lab work and imaging studies
- Prescribe medications and adjust treatment regimens as needed
- Collaborate with physicians and specialists to coordinate care
- Document patient progress and maintain accurate medical records
- Provide patient education on disease management and wellness strategies
- Monitor patient response to therapeutic interventions
- Participate in care planning meetings with interdisciplinary teams
- Ensure compliance with clinical protocols and regulatory standards
- Support quality improvement initiatives within the practice
- Maintain current licensure and certification requirements
- Respond to patient concerns and urgent clinical issues in a timely manner
- Conduct follow-up visits and adjust care plans accordingly
- Assist in training and mentoring support clinical staff
- Promote preventive care and health screening adherence
- Manage patient referrals to other specialists or services
- Uphold patient confidentiality and ethical standards
- Stay current with advancements in clinical practice and treatment guidelines
- Contribute to policy development related to patient care procedures
