CVS Health is seeking a full-time Nurse Practitioner or Physician Assistant to join our team providing In-Home Health Evaluations for Signify Health. In this role, you'll conduct comprehensive evaluations in members' homes, building trusted relationships and helping bridge gaps in care without prescribing medicine or altering treatment regimens.
What You'll Do
- Conduct In-Home Health Evaluations lasting approximately 45 minutes, including medication and medical history review and physical evaluation.
- Perform additional diagnostic tests if ordered by the health plan, such as diabetic eye exams or spirometry.
- Use an iPad and an intuitive clinical workflow for each evaluation.
- Engage with people one-on-one in their homes to answer health questions and provide education.
- Be flexible to travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products.
What We're Looking For
- Active, unrestricted license(s) in your coverage area(s).
- Board certification.
