Lola Blankets is hiring an Engagement Coordinator to be a vital first point of contact, introducing patients to and maintaining their connection with Strive's comprehensive care program. You will initiate relationships through outbound outreach and in-person meetings, connecting individuals with the right services and resources.
What You'll Do
- Proactively outreach to patients via phone to educate them on the care services available to them and connect them with the appropriate Strive resources.
- Meet or exceed performance targets defined as 'completed first visits' on a daily basis.
- Research and update patient information, leveraging internal and external data platforms, including EHRs and local market resources.
- Build key internal relationships to escalate patient support needs.
- Lead with empathy, gain trust, and develop an initial understanding of individual patient needs.
What We're Looking For
- Proven experience in a patient-facing, outreach, or coordination role within healthcare.
- Strong interpersonal and communication skills with the ability to build trust quickly.
- Ability to work independently to meet daily performance targets.
- Familiarity with data platforms and basic research to update patient information.
