Responsibilities
- Conduct initial and ongoing assessments of client needs and circumstances
- Develop and implement individualized care and service plans
- Coordinate access to housing, healthcare, and social services
- Maintain accurate and up-to-date client records
- Advocate for clients within healthcare and social service systems
- Provide referrals to community resources and support programs
- Monitor client progress and adjust plans as needed
- Engage clients through home visits and community outreach
- Support clients in achieving personal goals and milestones
- Collaborate with multidisciplinary teams and service providers
- Ensure compliance with program guidelines and regulations
- Assist clients with benefit enrollment and applications
- Promote client independence and self-sufficiency
- Respond to client crises and provide immediate support
- Facilitate access to mental health and substance use services
- Educate clients on available community programs
- Track outcomes and report on service delivery
- Participate in team meetings and case conferences
- Uphold client confidentiality and ethical standards
- Complete required documentation within established timelines
