Responsibilities
- Deliver virtual care management by phone or video to improve adult members' health outcomes through a coordinated, patient-focused approach.
- Collaborate with members and interdisciplinary care teams to develop meaningful care plans and support goal achievement.
- Assist members in understanding and managing complex medical conditions, treatment options, benefits, and healthcare processes.
- Coordinate with local healthcare providers to ensure seamless, integrated care delivery.
- Offer ongoing, empathetic follow-up care to build trust and support long-term health improvements.
- Support members and their families during acute health events, including hospitalizations and rehab stays, through structured case management.
- Arrange access to clinical and social resources that address members' holistic health needs.
