Job Summary
The Relief Social Work Resource Coordinator serves as an advocate and point of contact for assigned patients and families by supporting the Licensed Clinical Social Worker or Social Work Clinician in implementing the psychosocial treatment plan. The role involves assisting patients and families with resources throughout their medical treatment. The primary objective is to ensure access to appropriate resources, enhance patient experience, and help resolve complex resource challenges that might impede optimal patient care. The coordinator performs delegated tasks assigned by Social Workers, including coordinating resources and community assistance program referrals, and serves as an integral patient care team member.
Essential Functions
Employees must adhere to organizational standards including cultural sensitivity, patient rights, ethical treatment, safety protocols, and professional conduct.
* Collaborates with Social Workers and key healthcare professionals in patient care activities
* Attends interdisciplinary rounds and care conferences
* Conducts assessments and makes resource referrals
* Acts as primary liaison between healthcare facility and community resource agencies
* Maintains current resource database for Social Work Department
* Coordinates transportation, lodging, meals, and other necessary resources
* Maintains monthly accounting for distributed assistance
* Develops and implements new resource coordination programs
* Serves as communication bridge between clinical staff and families
Minimum Qualifications
Education: Bachelor's degree in health-related discipline from accredited institution
Experience: Three years of progressively responsible directly related work experience
Licensing: None required

Compensation Range
Hourly: $41.51 to $46.43
