Engage in meaningful dialogue with patients to gather a thorough assessment of their needs and challenges, covering physical, emotional, psychological, and social factors.
Develop a personalized care plan using a client-centered approach to address the patient's specific needs and goals.
Establish measurable goals to provide clarity, monitor progress, and ensure interventions and services are directed toward specific outcomes.
Collaborate with a multidisciplinary team to ensure a holistic and coordinated approach to the patient's care plan.
Provide ongoing support, guidance, and encouragement to help patients navigate challenges and achieve their goals.
Assist patients in identifying, referring to, and navigating appropriate community resources to help them reach their target goals.
Conduct regular follow-up visits, either in person or virtually, to assess progress, address emerging issues, and adjust the care plan as needed.

Completion of a Bachelor of Social Work from an accredited university.
Registered with the Ontario College of Social Workers & Social Service Workers.
Minimum of 5 years of experience working as a Social Worker.
Valid Ontario driver's license and a reliable vehicle.
Knowledge and experience working with a diverse patient population, including frail seniors and patients with multiple comorbidities.
Proficiency in computer software programs (e.g., Microsoft Office) and virtual communication platforms (e.g., Microsoft Teams, Zoom, GoToMeeting).

Hourly pay from $60 - $85 plus eligible travel pay
