Responsibilities
- Help clients with daily living activities such as bathing, dressing, grooming, toileting, and eating
- Adhere to personalized care plans and delegated care instructions
- Assist with mobility, including transferring and repositioning from bed, chair, or wheelchair
- Support medication reminders and follow care plan instructions according to agency policies
- Perform light housekeeping tasks and ensure a safe home environment
- Offer companionship, emotional support, and opportunities for social interaction
- Transport or accompany clients to appointments or errands when required
- Monitor, document, and report any changes in client condition to the clinical team
