The RN Hospitalist/Care Mgmt Liaison plays a central role in aligning clinical care with efficient patient transitions. Based at a faith-based facility in Orlando, this full-time day shift position ensures seamless coordination between Hospitalist providers and Care Management teams to support timely, effective discharge planning for complex cases.
What You'll Do
- Bridge communication between Hospitalist providers and Care Management to resolve challenges in patient care plans
- Conduct daily check-ins with providers to assess and adjust discharge strategies
- Engage in interdisciplinary discussions to refine care approaches and improve outcomes
- Share updates with clinical staff and implement changes to care plans as directed by physicians
- Step in when complications arise, offering guidance and support for high-acuity or difficult cases
Requirements
- Bachelor of Science in Nursing (BSN) required
- Active Registered Nurse (RN) license
- Minimum of three years of nursing experience
- Current Basic Life Support (BLS) certification
- Proven ability to manage fast-paced, high-pressure environments
- Strong critical thinking and problem-solving abilities
- Effective communication skills across diverse patient and provider groups
- Capacity to anticipate needs and act proactively
- Collaborative mindset with a focus on team-based care
