Responsibilities
- Evaluate patients' physical, emotional, cultural, and spiritual conditions using observation, interviews, and medical record reviews.
- Monitor patient responses and take appropriate actions to support positive health outcomes.
- Create and carry out individualized care plans while following nursing protocols and working with interdisciplinary professionals.
- Show proficiency in performing various clinical procedures such as IV therapy, wound management, medication delivery, catheter insertion, and specimen collection.
- Assess the readiness, needs, and capabilities of patients and families to provide effective health education.
- Deliver instructional support and assess how well patients understand and apply the information.
- Advocate for patient safety, well-being, and rights while collaborating within the care team.
- Maintain accurate, secure, and private documentation of patient health records.
- Record patient status, treatments, reactions, and results promptly and precisely.
- Utilize digital tools and systems to enhance communication, reduce errors, manage information, and assist clinical decisions.
