About the Role
This role involves providing high-quality, patient-centered care to individuals across Virginia via telehealth platforms. The practitioner will conduct virtual visits, assess patient needs, create treatment plans, and ensure continuity of care while adhering to clinical guidelines and regulatory standards.
Responsibilities
- Conduct virtual patient evaluations and follow-up appointments
- Assess medical histories and current health conditions
- Diagnose and treat acute and chronic conditions
- Develop individualized care plans based on clinical findings
- Prescribe medications in compliance with state regulations
- Order and interpret diagnostic tests as needed
- Educate patients on treatment options and preventive care
- Maintain accurate and timely electronic health records
- Collaborate with care coordination teams to support patient outcomes
- Ensure adherence to clinical protocols and safety standards
- Respond to patient messages within established timeframes
- Monitor patient progress and adjust treatment as necessary
