Responsibilities
- Perform initial and ongoing in-person evaluations to assess and address care requirements for individuals in senior living environments and similar settings
- Support advanced practice clinicians in creating and managing care strategies for chronic illnesses alongside the clinical care team
- Facilitate coordinated care delivery by working with primary care physicians, medical specialists, family members, designated representatives, and community-based services
- Collaborate with internal team members such as local care coordinators, partner success leads, and remote advanced practice providers
- Ensure thorough and precise recordkeeping in the organization's Electronic Medical Record system and related digital platforms
- Manage after-hours patient phone inquiries to assist with urgent care needs outside regular hours
- Serve as a clinical advisor and support resource for on-site personnel to enhance quality of care for residents
- Demonstrate adaptability and responsiveness in a dynamic role within an expanding healthcare startup environment
