Responsibilities
- Perform initial and ongoing health evaluations and in-person visits for seniors in residential care settings to assess and address medical needs.
- Support advanced practice clinicians in creating and managing care strategies for chronic illnesses alongside the clinical care team.
- Facilitate coordinated care delivery by working with primary care physicians, specialists, family members, POAs, and support services, including on-site community staff such as Health and Wellness Directors.
- Collaborate with internal team members including a local care coordinator, partner success lead, and remote advanced practice providers.
- Ensure thorough and precise recordkeeping in the organization's Electronic Medical Record system and related digital platforms.
- Respond to after-hours resident or caregiver phone inquiries, providing clinical triage and guidance as needed.
- Serve as a clinical advisor to on-site personnel, enhancing the quality of resident care through expert support.
- Adapt to dynamic responsibilities and evolving processes within a fast-growing startup environment.
