Responsibilities
- Perform initial and ongoing in-person evaluations and wellness checks for residents in senior living facilities and similar environments to assess and address healthcare needs.
- Support advanced practice clinicians in creating and executing care strategies for chronic illnesses, working alongside the clinical care team.
- Facilitate coordinated care by engaging with primary care physicians, medical specialists, family members, POAs, and community-based services to ensure continuity of care.
- Collaborate with internal team members such as Resident Care Coordinators, Partner Success leads, and remote Advanced Practice Providers.
- Ensure precise and thorough recordkeeping in the Electronic Medical Record system and other digital platforms used for patient management.
- Respond to after-hours resident or caregiver phone inquiries requiring clinical assessment and guidance.
- Serve as a clinical advisor to on-site personnel, enhancing the quality of resident care through expert input and support.
- Adapt to changing responsibilities and evolving processes within a dynamic startup environment.

Work Schedule
Full-time position with in-person duties across multiple senior care settings; limited remote work may be possible.
Travel Requirement
Must be willing and able to travel regularly between Pocatello and Chubbuck, Idaho.

Respond to after-hours patient phone calls requiring clinical triage and support.
