Responsibilities

Conduct various clinical assessments and screening instruments at key points throughout the patient care journey
Analyze assessment results to identify and prioritize patient needs effectively
Take ownership of creating and updating care plans in collaboration with a multidisciplinary team
Facilitate internal and co-led team rounds to ensure seamless and patient-centered care coordination
Master the use of Tuesday Health’s digital platforms and tools to support integrated care delivery
Develop genuine, trust-based relationships with patients through active listening and empathetic communication
Act as the central coordinator of a cross-functional care team focused on improving outcomes for patients with serious medical conditions
