Responsibilities
- Conduct various clinical assessments and screening instruments at key points throughout the patient care journey
- Analyze assessment results to identify and prioritize patient needs effectively
- Take ownership of creating and updating care plans in collaboration with a multidisciplinary team
- Facilitate internal and co-led team rounds to ensure seamless and patient-centered care coordination
- Master the use of Tuesday Health’s digital platforms and tools to support integrated care delivery
- Develop genuine, trust-based relationships with patients through active listening and empathetic communication
- Act as the central coordinator of a cross-functional care team focused on improving outcomes for patients with serious medical conditions
