About the Role
The Registered Dietitian will provide virtual nutrition assessments and therapeutic guidance to help patients manage chronic diseases. This role emphasizes collaboration with healthcare providers, documentation of care plans, and continuous patient engagement to improve health outcomes.
Responsibilities
- Conduct virtual nutrition evaluations and develop individualized care plans
- Deliver medical nutrition therapy for chronic conditions such as diabetes and heart disease
- Collaborate with physicians and care coordinators to support patient goals
- Document patient progress and adjust interventions as needed
- Educate patients on dietary modifications and lifestyle changes
- Respond to patient messages within established response time guidelines
- Participate in team huddles and care planning meetings
- Maintain up-to-date knowledge of clinical nutrition guidelines
- Support patient adherence through motivational interviewing techniques
- Ensure compliance with HIPAA and other regulatory standards
- Track time and services for accurate billing and reporting
- Engage in quality improvement initiatives
- Attend virtual training sessions and professional development opportunities
- Use electronic health record systems efficiently
- Provide culturally sensitive nutrition guidance
- Assist in refining clinical protocols based on patient outcomes
- Monitor patient lab results and clinical markers
- Communicate effectively with a diverse patient population
- Uphold patient confidentiality and professional ethics
- Support onboarding of new clinical team members
Nice to Have
- Master’s degree in nutrition, public health, or related field
- Certified Diabetes Care and Education Specialist (CDCES)
- Certification in Obesity Management
- Experience with complex patient populations
- Background in behavioral health nutrition
- Familiarity with population health tools
- Project leadership in clinical quality improvement
- Experience mentoring other dietitians
