Responsibilities
- Collaborate with nurse leadership to provide high-quality care for children with complex medical needs, including tracheostomy management, ventilation support, suctioning, cough assist, PEG/JEJ feeding, and medication administration.
- Provide care for a limited number of clients with conditions such as spinal muscular atrophy, cerebral palsy, brain injury, congenital muscular dystrophy, and other chronic health disorders.
- Develop trusting relationships with children and their families while delivering one-on-one care in home settings to support personalized care objectives.
- Coordinate with care coordination staff to ensure consistent, effective delivery of clinical services.
- Maintain a consistent and reliable work schedule as assigned.
