Key Responsibilities

Qualifications
- Bachelor’s degree in nursing, public health, or related clinical field
- Active Registered Nurse license in New York State
- Minimum of three years of clinical or population health experience
- Experience with care coordination or chronic disease management programs
- Proficiency in data analysis tools such as Excel, SQL, or Tableau
- Strong understanding of healthcare quality metrics and reporting
- Demonstrated ability to interpret clinical guidelines and research
- Excellent written and verbal communication skills
- Proven track record in leading team-based health initiatives
- Knowledge of HIPAA and patient privacy regulations
- Familiarity with value-based care models and reimbursement structures
