Responsibilities
- Provide therapy services to patients within a specified service area
- Assess patient needs for physical therapy in home environments and establish personalized treatment strategies
- Collaborate with physicians, patients, and family members to create care plans and set treatment objectives
- Design, organize, and carry out individualized therapeutic programs aimed at restoring functional abilities and fostering independence
- Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic methods
- Ensure timely and precise documentation for every patient visit
- Maintain clear communication with patients, families, and members of the care team, particularly during discharge planning
- Deliver services in accordance with agency guidelines and professional standards
