Responsibilities
- Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
- Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
- Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
- Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
- Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
- Manage documentation including clinical progress summaries and discharge planning.
- Identify needs for additional home health services and coordinate referrals as appropriate.
- Perform other duties as required for patient services.
