Responsibilities
- Assess physician medication orders for suitability, correct dosage, potential interactions, and proper administration methods.
- Offer clinical advice to physicians and healthcare staff on medication therapy adjustments and improvements.
- Keep patient/resident profiles current, including medical history, lab results, and all medications (prescription, over-the-counter, and home remedies).
- Ensure necessary lab tests for high-risk medications are ordered, reviewed, and communicated to the healthcare team.
- Work with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
- Record all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.
- Implement and enforce policies for the safe handling, dispensing, and clinical management of medications.
- Ensure adherence to all state and federal regulatory requirements related to pharmacy practice and medication management.
- Report medication errors and adverse drug events to the Pharmacy Manager as needed.
- Train and support the clinical team and healthcare agency staff on safe and effective medication handling procedures.
- Perform additional pharmacy-related tasks and responsibilities as assigned.
Requirements
- Bachelor’s Degree in Pharmacy
- One year of experience in long-term care
- Current and valid pharmacist license in the state of practice
- Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
- Proficiency in electronic medical records and pharmacy information systems
- Strong clinical judgment, and effective written and verbal communication
