PharMerica was looking for a Pharmacist

Responsibilities

  • Assess physician medication orders for suitability, correct dosage, potential interactions, and proper administration methods.
  • Offer clinical advice to physicians and healthcare staff on medication therapy adjustments and improvements.
  • Keep patient/resident profiles current, including medical history, lab results, and all medications (prescription, over-the-counter, and home remedies).
  • Ensure necessary lab tests for high-risk medications are ordered, reviewed, and communicated to the healthcare team.
  • Work with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
  • Record all communications and clinical interventions in the patient’s medical record, including interactions with families and caregivers.
  • Implement and enforce policies for the safe handling, dispensing, and clinical management of medications.
  • Ensure adherence to all state and federal regulatory requirements related to pharmacy practice and medication management.
  • Report medication errors and adverse drug events to the Pharmacy Manager as needed.
  • Train and support the clinical team and healthcare agency staff on safe and effective medication handling procedures.
  • Perform additional pharmacy-related tasks and responsibilities as assigned.

Requirements

  • Bachelor’s Degree in Pharmacy
  • One year of experience in long-term care
  • Current and valid pharmacist license in the state of practice
  • Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
  • Proficiency in electronic medical records and pharmacy information systems
  • Strong clinical judgment, and effective written and verbal communication
