About the Role
The position involves providing psychosocial support to patients and their families during end-of-life care, conducting assessments, facilitating care planning, and connecting clients with community resources.
Responsibilities
- Conduct psychosocial evaluations for patients and families
- Offer counseling to support emotional and mental well-being
- Collaborate with healthcare professionals on care plans
- Assist in identifying patient and family needs
- Provide crisis intervention when necessary
- Coordinate referrals to community services
- Document patient interactions and progress notes
- Support advance care planning discussions
- Maintain patient confidentiality per regulations
- Respond to on-call requests as scheduled
- Attend team meetings and case reviews
- Educate families on hospice services and processes
- Advocate for patient and family needs
- Monitor changes in patient psychosocial status
- Assist with grief and bereavement resources
Nice to Have
- LCSW licensure in Georgia
- Experience in hospice or palliative care
- Training in grief and bereavement counseling
- Familiarity with electronic health records
- Bilingual communication skills
