Responsibilities
- Conduct psychosocial evaluations for patients and families
- Provide emotional support during end-of-life transitions
- Develop individualized care plans with interdisciplinary input
- Facilitate advance care planning discussions
- Connect patients with community support services
- Document clinical interactions in a timely manner
- Respond to on-call patient needs as scheduled
- Coordinate discharge planning when applicable
- Advocate for patient and family needs
- Maintain compliance with regulatory standards
- Attend team care conferences regularly
- Support bereavement follow-up activities
- Educate families on hospice philosophy and services
- Assess for caregiver stress and coping strategies
- Identify and address social determinants of health
- Collaborate with nurses and chaplains on care goals
- Ensure patient confidentiality and ethical standards
- Participate in required clinical supervision
- Respond promptly to urgent psychosocial crises
- Maintain accurate and secure patient records
- Engage in ongoing professional development
- Support cultural sensitivity in care delivery
- Assist with transition planning for patients changing care levels
- Promote patient autonomy in decision-making
- Uphold organizational policies and procedures

Scheduling
