About the Role
The role involves delivering specialized palliative care to patients with serious illnesses, emphasizing pain and symptom control, care coordination, and emotional support for patients and families.
Responsibilities
- Assess and manage symptoms related to serious illness, including pain, anxiety, and fatigue
- Collaborate with physicians, social workers, and chaplains to develop individualized care plans
- Provide education to patients and families about disease progression and treatment options
- Support patients through end-of-life decision-making and advance care planning
- Conduct home and facility visits to monitor patient conditions
- Document patient assessments and care plans in electronic health records
- Respond to urgent patient and caregiver concerns in a timely manner
- Coordinate transitions of care between hospitals, hospice, and home settings
- Prescribe medications and adjust treatment plans based on patient response
- Promote patient dignity and comfort through holistic care approaches
- Participate in interdisciplinary team meetings to evaluate patient progress
- Ensure compliance with clinical protocols and regulatory standards
- Advocate for patient preferences in care delivery and treatment goals
- Maintain accurate and timely clinical documentation
- Support bereavement services for families after patient passing
- Engage in quality improvement initiatives related to palliative care delivery
- Stay current with best practices in palliative and end-of-life care
- Serve as a clinical resource for nursing and support staff
- Address ethical concerns related to patient care decisions
- Uphold patient confidentiality and privacy standards
Nice to Have
- Certification in hospice and palliative nursing (CHPN)
