About the Role
The LVN Case Manager will oversee patient care trajectories, conduct assessments, facilitate communication between care teams and patients, and ensure adherence to clinical protocols and documentation standards.
Responsibilities
- Conduct initial and ongoing patient assessments to determine care needs
- Develop and update individualized care plans in collaboration with clinical teams
- Monitor patient progress and adjust care strategies as necessary
- Serve as a primary point of contact for patients and their families
- Coordinate transitions of care across healthcare settings
- Ensure compliance with regulatory and organizational documentation requirements
- Communicate with physicians, social workers, and other providers
- Educate patients and caregivers on treatment plans and self-management
- Track and report key patient health metrics and outcomes
- Maintain accurate and timely electronic health records
- Support implementation of advance care planning initiatives
- Respond to patient inquiries and urgent care needs promptly
- Participate in care team meetings and case reviews
- Identify and escalate high-risk cases to supervising clinicians
- Promote patient engagement and adherence to care recommendations
- Utilize telehealth tools for remote patient monitoring and follow-up
- Ensure patient data privacy and HIPAA compliance
- Assist in quality improvement initiatives related to care delivery
- Stay current with clinical guidelines and best practices
- Contribute to training and onboarding of new care team members
