Responsibilities

Act as a point of contact between city, state, and international health agencies
Exchange patient data with other jurisdictions following established protocols
Process incoming and outgoing TB patient referrals promptly
Monitor the status of all patients referred to or from NYC
Enter complete and accurate interjurisdictional referral data into Maven
Verify that patients moving into or out of NYC during treatment are properly tracked and managed
Confirm accurate referral documentation and patient counts
Maintain and improve Maven data workflows for quality assurance
Coordinate with internal teams and external public health partners
Respond to TB-related inquiries from healthcare providers outside NYC
Perform additional tasks as assigned
