Responsibilities
- Visit patients' homes and community locations to deliver short-term, ongoing, and urgent pediatric nursing care.
- Develop empathetic, reliable connections with young patients and their families, ensuring care is aligned with cultural needs and coordinated across providers.
- Work alongside a multidisciplinary team including doctors, advanced practice providers, nurses, pharmacists, nutritionists, care navigators, social workers, and therapists.
- Perform and record in-home patient evaluations and carry out clinical procedures such as ear cleaning, rapid diagnostic testing, IV insertion, blood draws, wound management, and medication delivery.
- Educate patients and caregivers, coordinate follow-up plans, and manage care using a dedicated digital health platform.
- Transition as needed into a virtual nursing role to assess urgent pediatric cases remotely using standardized clinical guidelines.
- Record patient care activities in the electronic medical record (Athena) and maintain communication with families via a secure mobile application.
- Use digital health tools and virtual care platforms effectively to support timely and efficient treatment delivery.
