About the Role
The Hospital Care Coordinator ensures seamless patient transitions by managing care plans, connecting patients with resources, and supporting continuity of care across departments and discharge points.
Responsibilities
- Coordinate patient care activities during hospital stays and after discharge
- Collaborate with healthcare providers to develop individualized care plans
- Monitor patient progress and adjust care strategies as needed
- Facilitate communication between patients, families, and clinical teams
- Ensure timely referrals to specialists and community services
- Track patient outcomes and adherence to treatment plans
- Assist in identifying barriers to care and recommend solutions
- Support discharge planning to reduce readmission risks
- Educate patients on post-hospital care requirements
- Maintain accurate and up-to-date patient records
- Participate in care team meetings and case reviews
- Advocate for patient needs within the healthcare system
- Ensure compliance with care coordination protocols and standards
- Connect patients with social services and support programs
- Conduct follow-up calls or visits as required
- Assist in care transitions between departments or facilities
- Identify high-risk patients for targeted interventions
- Use health information systems to manage patient data
- Support quality improvement initiatives in care delivery
- Promote patient engagement in their treatment plans
