What You’ll Do • Provide direct, skilled nursing care in patients’ homes under the supervision of an RN, following physician-ordered care plans • Perform clinical treatments including wound care, medication administration, injections, IV therapy, and vital signs monitoring • Observe and assess patient conditions, promptly reporting any changes to the RN or physician • Collaborate with RN Case Managers and interdisciplinary teams to support and update individualized care plans • Educate patients and families on disease management, medications, safety, and self-care techniques • Supervise and support Home Health Aides, ensuring quality and consistency of care • Maintain accurate, timely clinical documentation in compliance with agency, state, and payer requirements • Participate in case conferences and quality improvement initiatives • Uphold strict infection control and safety standards during all patient interactions • Efficiently manage daily travel and visit schedules within your assigned territory What You Bring • At least 1 year of clinical nursing experience and 1 year of home health experience • Strong ability to follow physician-directed treatment plans and maintain thorough, accurate medical records • Excellent communication and interpersonal skills with a patient-first mindset • Ability to work independently while collaborating effectively with a care team Required Credentials • Graduate of an accredited Practical or Vocational Nursing program • Active LPN/LVN license in the state of practice • Current CPR certification (must be maintained throughout employment) • Valid driver’s license and reliable transportation to travel between patient homes
