Responsibilities
- Provide in-home physical therapy services within an assigned geographic region
- Assess patients' therapy needs in their home environment and create personalized treatment strategies
- Collaborate with physicians, patients, and family members to establish care plans and therapeutic goals
- Design and carry out individualized rehabilitation programs aimed at restoring mobility and functional independence
- Educate caregivers, family members, physical therapist assistants, and home health aides on proper therapeutic methods
- Ensure accurate and prompt documentation of all patient visits
- Maintain clear communication with patients, families, and healthcare team members, particularly during care transitions and discharge planning
- Deliver services in alignment with agency protocols and professional standards
